چکیده
A 78-year-old diabetic male was admitted with jaundice, weight loss, and intermittent upper abdominal pain. Laboratory tests disclosed anaemia (haemoglobin 9.4 mg/dL), elevated C-reactive protein, serum bilirubin (17 mg/dl), transaminase and alkaline phosphatase levels. Following ultrasound confirmation of biliary dilatation, magnetic resonance cholangiopancreatography (MRCP) depicted a concentric stenosis of the proximal common bile duct (CBP) (arrowhead, Fig. 1A), with moderate upstream dilatation of common hepatic duct and intrahepatic branches, suggestive of a neoplastic nature. Endoscopy showed a normal-appearing Vaterian papilla, which was cannulated without the need for sphincterotomy. Contrastmedium injection con rmed the MRCP nding of CBD stricture causing upstream dilatation. After cytological brushing, a 10-cm 10-French plastic biliary stent was positioned, without immediate adverse events. Twenty-four hours later the patient complained of acute di use abdominal pain, without signs of peritonitis at physical examination. Urgent contrast-enhanced multidetector CT (MDCT) showed correctly placed biliary stent, moderate ascites and marked circumferential mural thickening of the duodenum from the papilla to the Treitz angle with strati ed appearance (“target sign”) by enhanced mucosa and hypoattenuated oedematous submucosa (arrows), indicative of mucosal in ammation and oedematous submucosa (Figs. 1B, C), as well as minimal associated perivisceral fat in ammatory changes. Imaging ndings and laboratory data were inconsistent with post-procedural acute pancreatitis, haemorrhage or duodenal perforation. e patient promptly improved with conservative treatment, and jaundice was e ectively relieved. Brush cytology diagnosed CBD adenocarcinoma. Paracentesis uid analysis excluded infection and malignancy. ree weeks later, repeated ERCP showed a normal endoscopic aspect of the duodenum. Management of endoscopic biliary stenting for choledocholithiasis: Evaluation of stent-exchange intervals
AIM To evaluate the best management of plastic stents in patients with choledocholithiasis who were unfit for endoscopic stone removal or surgery. METHODS Between April 2007 and September 2017, 87 patients (median age 83.7 years) with symptomatic choledocholithiasis were treated with insertion of 7-Fr plastic stents because complete endoscopic stone retrieval was difficult, and their general ...
متن کاملRevision of biliary sphincterotomy by re-cut, balloon dilation or temporary stenting: comparison of clinical outcome and complication rate (with video)
Background and study aims Revision of endoscopic retrograde cholangiopancreatography (ERCP) may be necessary following previous biliary endoscopic sphincterotomy for recurrent biliary symptoms related to biliary stone recurrence, cholangitis or post-biliary endoscopic sphincterotomy (bEST) papillary stenosis and cholestasis. The aim of this retrospective study was to evaluate the clinical outco...

Objective: Bilary stenting with endoprosthesis is a palliative prodedure to relieve malignant biliary obstruction. Percutaneous transhepatic technique has been employed whenever endoscopic technique was not possible or had fialed. متن کاملPerforation and abscess formation after radiological placement of a retrievable plastic biliary stent
INTRODUCTION Retrievable plastic biliary stents are usually inserted endoscopically. When endoscopic placement fails, radiological percutaneous transhepatic placement is indicated. We report the occurrence of a case of delayed duodenal perforation with abscess formation after radiological placement of a plastic stent. To the best of our knowledge, this is the first report of this complication a...
متن کاملPartially covered self-expandable metallic stents for benign biliary strictures due to chronic pancreatitis.
BACKGROUND AND STUDY AIMS Benign biliary strictures (BBS) may occur in patients with chronic pancreatitis and may lead to secondary biliary cirrhosis or recurrent cholangitis. Although surgical diversion may provide definitive therapy, it can be associated with significant morbidity. Endoscopic therapy with plastic stents has been used as an alternative to surgery but has resulted in unsatisfac...
